Per IRS requirement, a Social Security Number or Federal Identification Number is required for payment of premiums and must be obtained by completing, signing & returning a W-9 form to the Kansas State Fair Administration Office, include information when completing an online entry or providing the information on a paper entry form AND signing the form verifying it is true and accurate information for the exhibitor (not parents number if the exhibitor is a minor). Exhibitors who choose to not provide their Social Security numbers will forfeit their premiums.
